This is the basic template to follow when creating your character. Feel free to add more information, but this is what is required.
Name: Your full name. First and last name, middle if you wish.
Gang: If applicable, what gang are they part of?
Ethnicity: What ethnicity are they?
Age: Anything reasonable. Can be as young as 13, to 40-something, at most.
Gender: Male or female.
Height: How tall your character is. They can be a giant, or they can be small.
Weight: How much your character weighs.
Eye Color: What is their natural eye color? If they were contacts, include that too.
Hair Color: What is their natural hair color. If they dye their hair, include that too.
Appearance: A picture works fine, but we don't always see everything. Does your character have scars, or birth marks? A little detail of how your character looks is always nice.
Personality: This is one of two sections that staff will be hard on. Your character's personality is one of two things that can give other members a view of how your character is, so at the absolute minimum, you must give a basic rundown of your characters psyche. What are they like on the inside, caring people, cruel, slightly ambivalent, whatever, it's your character. You must write something for this that matches your history, considering personalities are forged by life experience.
History: This is the second section that staff will be hard on. Detail is the key, make it a good length, at the bare minimum, three full paragraphs. These must, at some point or another, detail your character's early childhood and home life (relevance to personality and other valuable information), your character's younger years and any such thing that encompasses the years approaching their current age. Describe important life events and such things, that helped shape who your character is today. The last part must describe where and what your character is doing presently.

Your character's stats! You have 75 EXP to distribute between the five categories as you please! No more than 20 in any category, no less than 5. If your character is a transfer, you must try to post proof of their EXP gains.

Bio Approval Phrase: This is located in the rules section of the board. You cannot be approved without it, because this proves you have read and understand the rules, and are responsible for your actions on this board and the consequences attached.
